Risk is an inherent part of daily aviation operations. However, through vigilance, hazard awareness, and a continuous learning process, such risks can be effectively managed and reduced. Incident reporting plays a key role in preventing recurrence.
The Importance of Reporting
To ensure the ongoing improvement of aviation safety, it is essential to collect comprehensive information on past events. Each report contributes to the identification of potential risks, the refinement of risk assessments, and the analysis of root causes and contributing factors. These insights support targeted safety enhancements and risk mitigation across Swiss aviation.
All occurrences—regardless of whether they are minor or major—offer valuable data that help avoid the repetition of past errors and contribute to a stronger safety culture.
A Simplified Reporting Process
To facilitate clear and straightforward reporting, the Federal Office of Civil Aviation (FOCA) has developed a compact, pocket-sized reference card. This card outlines the steps to be taken following an incident, regardless of its severity.
In case of an accident (definition accident) or serious incident (definition serious incident), the Swiss Transportation Safety Investigation Board (STSB) must be notified immediately by calling 1414.
Although this number also connects to REGA, it serves as the main contact point for the STSB, which coordinates the subsequent investigation process.
In addition to contacting 1414, the occurrence must be reported to FOCA within 72 hours via the ECCAIRS2 aviation reporting portal. This applies to all types of incidents regardless of the severity. To streamline this process, the reporting form is available in English, German, French, and Italian.
Advantages of Using ECCAIRS2
Creating an ECCAIRS2 account allows personal information to be pre-filled automatically, significantly reducing reporting time. It also enables access to submitted reports for follow-up or updates—such as new information that may emerge following technical investigations or maintenance.
Anonymous submissions are also possible. If contact information is provided, a confirmation email is sent with access to the submitted report.
What Happens After a Report Is Submitted?
Each report is reviewed and categorized by FOCA based on severity. If no further action is required, the occurrence is recorded in the safety database for future analysis.
